- Lamotrigine
- Lamotrigine half-life rises from 26 to 70 hours. Reduce the lamotrigine dose; serious skin reactions have occurred.
- Phenytoin
- Valproate displaces phenytoin and slows its metabolism, raising free phenytoin. Phenytoin, carbamazepine, and phenobarbital can double valproate clearance.
- Carbapenems (meropenem, imipenem, ertapenem)
- Can drop valproate to subtherapeutic levels and cause seizures. Monitor levels or choose another drug.
- Topiramate
- Hyperammonemia with or without encephalopathy, and hypothermia.
- Aspirin, estrogen-containing contraceptives
- Alter valproate levels. Monitor concentrations.
- Warfarin
- Raises unbound warfarin. Monitor coagulation tests.
