- Potassium supplements, salt substitutes, ACE inhibitors, ARBs, NSAIDs, heparin, trimethoprim
- Severe hyperkalemia possible. Usually stop potassium supplements in heart failure; recheck potassium when ACE inhibitor or ARB doses change.
- Lithium
- Less lithium clearance and toxicity risk. Monitor lithium levels.
- NSAIDs or aspirin
- Can blunt the diuretic and blood pressure effect; aspirin may require a higher spironolactone dose.
- Digoxin
- Can falsely raise digoxin results on some immunoassays. Use an assay that does not cross-react.
- Cholestyramine
- Hyperkalemic metabolic acidosis has been reported.
- Abiraterone or mitotane
- May raise PSA with abiraterone and lowers mitotane levels. Avoid these combinations.
