- Tachycardia and hypertension
- More norepinephrine at synapses increases sympathetic tone. Management: Screen for cardiac disease before starting; monitor blood pressure and pulse.
- Insomnia and appetite loss
- Central stimulation from increased dopamine and norepinephrine. Management: Give doses before meals and the last dose before 6 p.m.; track weight.
- Growth suppression
- Reduced appetite and intake contribute; the full mechanism is not established. Management: Monitor height and weight; interrupt treatment if growth falls off.
- Abuse and addiction
- Dopamine increases in reward pathways reinforce use, especially with snorting or injection. Management: Assess risk, store securely, and monitor for misuse.
- Peripheral vasculopathy and Raynaud phenomenon
- Adrenergic vasoconstriction in the digits. Management: Watch fingers and toes; lower the dose or stop if signs appear.
- Hypertensive crisis with MAOIs
- MAOIs block breakdown of the extra norepinephrine and dopamine released. Management: Never combine, and wait 14 days after an MAOI.
