The literature is large but lopsided: cell culture, animal, and biomarker studies plus older regional human reports, nearly all from the Saint Petersburg group that developed the peptide. The strongest human claim, lower mortality over 6 to 8 years in 266 elderly people, used the pineal extract Epithalamin rather than synthetic epitalon and was not blinded. Rat studies showed lifespan effects only under abnormal lighting. The 2025 cell-line paper is the first to quantify the telomerase pathway in different cell types.
No independent, modern, controlled trial of synthetic epitalon in humans has been published. Whether telomere changes in cells mean anything for aging in a person is itself contested, and the cancer-cell findings raise a safety question no trial has addressed. Product identity is central, since the extract studied decades ago, the synthetic peptide, and the vials sold online are three different things. The July 2026 advisory vote was about compounding rather than approval and added no new trial evidence.
