- CYP3A4 inhibitors: cimetidine, macrolides (erythromycin, clarithromycin), azole antifungals, diltiazem, verapamil, fluoxetine, isoniazid, protease inhibitors, grapefruit juice
- Raise carbamazepine levels and toxicity risk. Monitor levels closely and adjust the dose.
- Epoxide hydrolase inhibitors: valproic acid, quetiapine, loxapine, brivaracetam
- Raise the active epoxide metabolite. Adjust the dose or monitor levels.
- CYP3A4 inducers: phenytoin, fosphenytoin, phenobarbital, primidone, rifampin, felbamate, theophylline
- Lower carbamazepine levels; risk of breakthrough seizures.
- Hormonal contraceptives (oral and implant)
- Lower hormone levels; breakthrough bleeding and unintended pregnancy reported. Use a backup or alternative method.
- Rivaroxaban, apixaban, dabigatran, edoxaban
- Anticoagulant levels may be too low to work. Generally avoid the combination.
- Drugs weakened by enzyme induction: warfarin, lamotrigine, phenytoin, valproate, clozapine, haloperidol, aripiprazole (double its dose), tricyclic antidepressants, corticosteroids, cyclosporine, tacrolimus, methadone
- Lower levels and reduced effect. Monitor levels or adjust doses.
- MAO inhibitors; nefazodone
- MAO inhibitors are not recommended (stop at least 14 days before). Nefazodone is contraindicated because its levels become too low to work.
- Lithium
- Higher risk of neurotoxic side effects.
- Isoniazid
- Raises carbamazepine levels and increases isoniazid liver toxicity.
- Nondepolarizing neuromuscular blockers (rocuronium, vecuronium, pancuronium, cisatracurium)
- Resistance to blockade; recovery may be faster than expected.
- Liquid chlorpromazine or thioridazine, or any other liquid medicine
- Mixing with the suspension forms a rubbery precipitate. Do not give the suspension at the same time as other liquids.
- Alcohol
- Additive sedation.
