A 2025 systematic review found 36 studies, 35 preclinical and 1 clinical, with consistent improvements in animal muscle, tendon, ligament, and bone injury models. The human record is a chart review in which 11 of 12 patients given knee injections reported relief, a 12-woman bladder pilot in which all reported symptom resolution, and a 2-person intravenous study with normal blood tests afterward. None was blinded, randomized, or controlled, and two were run at the same private clinic. A 2015 oral safety study in healthy volunteers in Mexico lists no results.
The biggest unknown is whether a standardized BPC-157 product can show benefit in a controlled human trial, which the 2026 hamstring study will be the first to test. Product identity is a separate problem, since the material in rat studies, the compounded peptide, and internet vials have never been shown to match. Long-term safety, immune reactions, and dosing in people are unmeasured. Until the trial reports, every human claim rests on animals or on uncontrolled reports.
