WarfarinNursing considerations

Known brand names include Jantoven and Coumadin.

An oral blood thinner that slows clot formation. It prevents and treats clots in the legs and lungs and prevents stroke from atrial fibrillation or artificial heart valves. Regular INR blood tests guide the dose.

Nursing implications

Hold parameters

Stop and reverse for serious bleeding
INR above 4.0 adds bleeding risk without added benefit
Stop if skin necrosis, calciphylaxis, or cholesterol emboli develop
Do not start during acute HIT until the platelet count has normalized
Dental or surgical procedures may require interruption or a dose change

Administration

High-alert: once daily, dose set by INR
Missed dose: take it the same day as soon as remembered; never double the next day
Routine loading doses are not recommended
When converting from heparin, overlap for 4 to 5 days and until INR is therapeutic
During heparin overlap, draw INR at least 5 hours after an IV heparin bolus, 4 hours after stopping an infusion, or 24 hours after a subcutaneous dose

Bleeding

Bleeding: bruising, nosebleeds, bleeding gums, heavy periods
Major or fatal hemorrhage, most often in the first month (serious)
Limb gangrene if started during acute HIT (serious)

Skin and allergy

Skin or tissue necrosis, usually within days of starting (serious)
Calciphylaxis (serious)
Purple toes from cholesterol microemboli (serious)

Renal and urine output

Acute kidney injury with excessive anticoagulation and hematuria (serious)

Labs and vitals

aPTT (partial thromboplastin time)

Why: Warfarin can prolong aPTT even without heparin
When: Around surgery
Act on: aPTT above 50 seconds with an in-range INR signals higher postoperative bleeding risk
Normal: 25 to 35 seconds when not on a blood thinner.

INR (prothrombin time)

Why: Therapeutic anticoagulation and bleeding risk
When: Daily from the first dose until stable, then every 1 to 4 weeks; extra checks when other drugs, diet, or brands change
Act on: Most targets 2.0 to 3.0 (2.5 to 3.5 for some mechanical valves). Above 4.0 adds bleeding risk without benefit
Normal: INR 0.8 to 1.1 when not on a blood thinner. On warfarin, the usual target is 2.0 to 3.0.

Platelet count

Why: Timing warfarin start after heparin-induced thrombocytopenia
When: Before starting in patients with HIT
Act on: Start only after the platelet count normalizes
Normal: 150,000 to 400,000 per microliter.

High-alert status

High-alert medication. Warfarin is named on the ISMP acute care high-alert list under anticoagulants.

Narrow therapeutic index. The label states that warfarin has a narrow therapeutic range, so dosing is guided by INR.

Boxed warning

Bleeding risk. Warfarin can cause major or fatal bleeding. Monitor INR regularly in everyone. Drugs, diet, and other factors change the INR, and patients must be taught how to prevent and report bleeding.

Antidote or reversal

Vitamin K1 (phytonadione), oral or IV, after stopping warfarin. For urgent reversal in life-threatening bleeding: prothrombin complex concentrate, fresh frozen plasma, or activated factor VII, which carry thrombosis and infection risks.

Patient teaching

Take it exactly as prescribed and keep every INR appointment
Keep vitamin K intake steady; avoid sudden large changes such as many more leafy greens
Report unusual bleeding: prolonged bleeding from cuts, nosebleeds, bleeding gums, red or dark urine, black stools, heavier periods, headache, or dizziness
Do not start or stop any medicine, aspirin, or herbal product without asking
Tell every provider and dentist, and carry ID saying you take warfarin
Avoid activities with a high risk of injury, and report frequent falls
Report painful purple skin areas or purple toes right away
Avoid pregnancy; use effective birth control

Dosing

Doses come from the FDA label. Study tool, not for clinical decisions.

Adult dose

Starting (genotype unknown): 2 to 5 mg once daily, then adjust by INR. Typical maintenance is 2 to 10 mg daily. Use lower doses for older, debilitated, or Asian patients.
VTE, atrial fibrillation, bioprosthetic mitral valve: Target INR 2.5 (range 2.0 to 3.0). VTE with a reversible cause: 3 months; unprovoked: at least 3 months, then reassess.
Mechanical mitral valve or caged-ball/caged-disk valve: Target INR 3.0 (range 2.5 to 3.5).
High-risk post-MI: INR 2.0 to 3.0 plus aspirin up to 100 mg/day for at least 3 months.

Renal adjustment

No dose change, since renal clearance plays little role. Check INR more often in renal impairment.

Hepatic adjustment

No set dose change, but liver disease strengthens the effect through lower clotting factor synthesis and slower warfarin metabolism. Watch for bleeding more often.

How it works

Mechanism

A vitamin K antagonist. It blocks VKORC1, so vitamin K cannot be recycled, and the liver makes less functional factors II, VII, IX, and X and proteins C and S. Full effect takes several days, and the effect persists 2 to 5 days after stopping.
