- Reflex tachycardia, palpitations, and angina
- Arteriolar dilation triggers reflex sympathetic discharge, which raises heart rate and myocardial oxygen demand. Management: Avoid in coronary artery disease; a beta blocker is often added to blunt the reflex.
- Sodium and fluid retention, edema
- Reflex renin release activates angiotensin II and aldosterone. Management: Combine with a diuretic; watch weight and edema.
- Drug-induced lupus
- Immune reaction that is more common at high doses and in slow acetylators, who have higher drug levels. Management: Check CBC and ANA before and during long-term therapy; stop the drug if lupus-like symptoms develop; steroids may be needed.
- Peripheral neuritis
- An antipyridoxine (vitamin B6) effect. Management: Add pyridoxine if symptoms develop.
- Headache and flushing
- Direct vasodilation. Management: Start low and increase gradually.
