Direct factor Xa inhibitors
Direct factor Xa inhibitors block factor Xa, a key step in the clotting cascade. Apixaban and rivaroxaban prevent stroke in atrial fibrillation and treat deep vein thrombosis and pulmonary embolism. They are given at fixed doses without routine INR checks and have fewer food interactions than warfarin. Bleeding is the main risk, and andexanet alfa can reverse their effect in a major bleed.
