- Nausea, vomiting, diarrhea, anorexia
- Increased cholinergic stimulation of the GI tract. Management: Titrate slowly (a 6-week titration to 10 mg had lower rates than 1 week), watch intake and weight.
- Bradycardia, heart block, syncope
- Vagotonic effects of extra acetylcholine on the SA and AV nodes. Management: Check pulse, review other rate-slowing drugs, and evaluate any syncope.
- Peptic ulcer and GI bleeding
- Cholinergic stimulation increases gastric acid secretion. Management: Use caution with ulcer history or NSAIDs; watch for bleeding.
- Insomnia and abnormal dreams
- Central cholinergic stimulation at night. Management: Consider morning dosing if bothersome.
- Muscle cramps
- More acetylcholine at the neuromuscular junction. Management: Usually transient; reassess dose if persistent.
- Bronchospasm in asthma or COPD
- Cholinergic bronchoconstriction and increased airway secretions. Management: Prescribe with care and monitor breathing.
- Urinary retention
- Cholinomimetic effects can cause bladder outflow obstruction. Management: Monitor urination, especially with prostate enlargement.
