- Omeprazole or esomeprazole (CYP2C19 inhibitors)
- Markedly reduce antiplatelet activity. Avoid. Pantoprazole, lansoprazole, and dexlansoprazole have less effect.
- Strong CYP2C19 inducers such as rifampin
- More active metabolite and more bleeding risk. Avoid.
- Opioids such as morphine
- Delay and reduce clopidogrel absorption. In ACS, consider a parenteral antiplatelet agent.
- NSAIDs, warfarin, SSRIs and SNRIs, other antiplatelet agents
- Raise bleeding risk, including GI bleeding.
- Repaglinide (CYP2C8 substrate)
- Exposure rises about 4- to 5-fold. Avoid; if needed, start at 0.5 mg before meals, at most 4 mg per day, and check glucose more often.
