- Thiazide diuretics
- Higher risk of hypersensitivity and severe rash, especially with renal impairment. Monitor kidney function and lower the allopurinol dose.
- Ampicillin, amoxicillin, or bendamustine
- Higher risk of skin rash, which may be severe.
- Mercaptopurine or azathioprine
- Allopurinol blocks their breakdown and raises exposure and myelosuppression risk. With 300 to 600 mg/day of allopurinol, cut their dose to about one third to one fourth.
- Warfarin or dicumarol
- Anticoagulant effect may increase. Check INR or prothrombin time often and adjust.
- Cyclosporine
- Cyclosporine levels rise. Monitor levels more often and adjust.
- Capecitabine or fluorouracil
- Their anticancer effect may fall. Avoid the combination.
- Pegloticase
- May blunt the uric acid rise used to monitor pegloticase safely and raise the risk of pegloticase anaphylaxis. Do not use allopurinol during pegloticase.
- Theophylline
- Allopurinol at 600 mg/day or more may lower theophylline clearance. Monitor and adjust.
- Cyclophosphamide and other cytotoxic drugs
- More bone marrow suppression. Monitor blood counts more often.
- Uricosuric drugs
- Lower oxypurinol exposure. Monitor uric acid.
- Alcohol and other CNS depressants
- Additive drowsiness.
